  • Support Navigation

    Navigating the many services potentially available here in Summit County to individuals and families in their last chapter can be daunting. Our Care Coordinator will visit your home to discuss available options and develop a plan that will include both lean on us support services as well as other community resources you or your loved one might be eligible for. We will walk side by side with you during your family’s last chapter.

  • Care Provider Relief/Respite

    Caring for a loved one in their last chapter can become a 24 hour commitment which drains both the physical and emotional energy of the provider. lean on us will seek to help mitigate this stress through providing various services such as meals, cleaning, companionship, transportation, or emergency financial assistance. The intent is to provide time for the caregiver and family to solely focus on their loved one.

  • Meals

    Time with a loved one is of the essence during those last few months or days. Volunteers delivering home cooked meals can provide much needed nourishment as well as freeing up family members to focus on their loved ones.

  • Transportation

    Often because the caregiver or patient is focusing on the needs of the situation at hand, they do not have the time or capacity to run their daily errands. A cadre of volunteers will be able to assist, driving to the store for groceries, picking up mail or transporting a patient to the doctors, to name a few.

  • Hospice Counseling

    When the time comes, lean on us will work very closely with Bristlecone Hospice to help provide individuals and their family, hospice support that is timely, caring and patient focused.

  • Companionship

    Caregiving is an around the clock activity which is draining both physically and emotionally. Our trained volunteers will provide companionship for loved ones so that the caregiver can have time for their own self-care. In the case where an individual is by themselves, lean on us volunteers can do welfare checks or spend time supporting these individuals during their journey.

  • Emergency Financial Support

    In many cases, due to the loss of income or the cost of medical care, individuals or families are in need of short term financial support. Whether it’s rent, a mortgage payment, food, care or prescriptions, lean on us, within its available funding, will try to help mitigate the impacts. This all because of the many generous donors across Summit County.

  • Contractor References

    Nearing the end of the last chapter, all focus turns to the loved one, but day-to-day life still continues. Things break, faucets leak, the house needs cleaning, or it’s time for some form of home health care support. Given these normal needs, lean on us will have a group of service providers available that have been vetted and provide a discount to our clients.
