Eligibility/ Referral Requirements
In order to be eligible for services the patient must be a resident of Summit County and have a life ending condition that can be certified by a physician. Once these requirements have been met our Support Navigator will call to schedule a visit at the individual’s home to determine the level of services needed and final eligibility.
Referrals can come from Doctors, Social Workers, Clergy, Government agencies, other Social Service organizations, and patients or their families. The attached form needs to be completed and submitted to lean on us in order to start the eligibility process cited above.
